The successful candidate should have a residency or five years experience in a decentralized role; anticoagulation experience is also required (CACP Certification a plus). Facilitates the safe and effective use of anticoagulation therapy by providing patient education, consultation, therapeutic monitoring for efficacy, safety, evaluation drug interaction potential, making appropriate dosage adjustments and effectively communicating to the patient and/or care-provider, physician and the healthcare team. Assures appropriate, complete and accurate documentation in the Patient's Medical Record. Oversee the daily operations, planning, collecting data and identifying performance improvement opportunities. Supervises the work of technical and clerical support personnel in the clinic setting.
B.S. or Pharm. D., Ohio License, ASHP Residency or 5+ years in a decentralized role, plus anticoagulation experience.

In Southern NH the Nashua Medical Group Practice seeks an NP to manage their anticoagulation program for a busy Adult Medicine Practice. We are seeking a qualified (RN, NP) to join their friendly multidisciplinary team. The candidate will be primarily responsible for management of the Anticoagulation Clinic. This clinic services approximately 150 anticoagulation patients. The (RN, NP) will provide management of anticoagulation medication(s) during scheduled patient visits and over the telephone.
The appropriate candidate will have knowledge of goals of anticoagulation therapy as well as understanding of the food, drug and disease interactions that make anticoagulation therapy challenging. We are a progressive medical practice which uses a full electronic medical record for documentation and billing purposes. Excellent written and oral skills are required and experience in using computer systems to document encounters is a plus. The candidate must be able to work independently and use clinical skills to create and enhance anticoagulation management protocols.

We are looking for an enthusiastic per diem pharmacist to supplement our team of providers on a per diem basis in The Polyclinic Anticoagulation Clinic. Our clinic services about 500 ambulatory patients, providing management of Warfarin and LMWH via scheduled patient visits and use of point-of-care INR testing. Our team consists of three part-time pharmacists, an ARNP, and two full time medical assistants.
Responsibilities:
• Clinical assessment of patients regarding indication for anticoagulation therapies
• Warfarin dosage adjustment per protocol
• Management of anticoagulation therapies around procedures including use of LMWH as bridge therapy per protocol
• Evaluate patients for safe and effective use of anticoagulation therapy
• Documentation of patient visits in electronic database and clinic medical record
• Provide refill authorization for anticoagulation therapies as appropriate
• Provide ongoing education for patients and providers
• Act as patient advocate in communicating with other healthcare providers
• Adapt and perform effectively in a continuously changing environment
• Participate and contribute in a team-based environment
• Understand and deliver quality care and service consistent with the Polyclinic’s efforts in becoming a higher performance organization
Qualifications:
Graduate of an accredited school of pharmacy
Certificate/Licensure:

The Anticoagulation Management Services at MGH is looking for a dynamic Staff Nurse to join their team! You will work with the collaborating physician to enhance anticoagulation management for his/her primary patients across the continuum. Use your experience and clinical expertise as a pivotal partner with patients, nurses, physicians, case managers, and pharmacists to facilitate implementation of an anticoagulation plan of care. In this role, you will focus on monitoring patients' INR values and managing anticoagulation medication(s) to maintain a prescribed therapeutic range. Additionally, patient education, care and support during one-on-one visits and over the telephone is provided. This position requires a RN with energy, enthusiasm and the ability to manage multiple priorities. The clinic is staffed between 8:00AM and 5:00PM weekdays, and 8:00AM to 4:30PM weekends and holidays.
Requirements:
Must be baccalaureate prepared registered nurse (MSN preferred) licensed in Massachusetts with a minimum 5 years direct care experience. Acute care hospital experience; cardiac, neurology, general medicine, and knowledge of the goals of anticoagulant therapy a plus. You must be a well organized nursing professional with the excellent written and verbal communication skills needed to provide daily updates and instructions to patients, family members and other caregivers.
